Take Care Utah, a program of UHPP, the United Way of Salt Lake, and 2-1-1 partners with a variety of organizations in your neighborhood and around the state to help children get and keep affordable health care coverage—and use their benefits wisely.
Why is it important to get all kids enrolled?
Many children qualify for Medicaid or CHIP today but are not yet enrolled! Help us achieve our mission of substantially lowering the uninsured rate in your area by engaging those populations that are in the most vulnerable such as low-income communities. Remember, healthy kids make better learners!
Who qualifies for Medicaid/CHIP and other programs?
The Department of Workforce Services determines eligibility for Medicaid/CHIP, and Utah Premium Partnership, a program that helps families pay for health insurance through their employer. You can help identify uninsured families, encourage them to apply, and help them get the paperwork ready. Download this flyer template.
What does the enrollment process look like?
You can help families through the enrollment process by building trust and sharing information along the way. The process includes:
- Outreach to families
- Assistance with filling out the enrollment form and gathering all required documents
- Follow-up during the process

I’ve heard that the Medicaid database was breached and all the Social Security Numbers were stolen. What does that mean to anyone enrolling now?
The Department of Health is working hard to recover from this unfortunate incident. Right now the best way to keep up on the latest developments is to visit the Department of Health website 's data breach page.

First Steps to the Enrollment Process
